Cellulite Treatment: Causes, Methods and Most Effective Solution

By Prof. Dr Ahmet Karacalar

What is Cellulite?

An effective cellulite treatment Firstly, it is necessary to define the problem correctly for the process. Cellulite (Gynoid Lipodystrophy) is the appearance of “orange peel” on the skin by squeezing the fat tissue under the skin between the connective tissue bands. It is not only a weight problem; it is a condition based on genetic and hormonal structure.

Prof. Dr Ahmet Karacalar; a successful treatment rejects a “uniform” approach. Success in treatment depends on planning according to the stage and type of cellulite (hard, soft) and personalised anatomical structure.

Cellulite is Not Just Cellulite

The orange peel appearance is a reflection of underlying metabolic problems.
It therefore requires a medical perspective.

Lipoedema, These include insulin resistance, chronic inflammation, estrogen dominance, mitochondrial dysfunction, microbiota dysfunction and deficiency of certain vitamins.
After solving these, superficial layer liposuction and cutting the bands are required for stage 3 and 4 cellulite. If there is a problem due to tissue sagging, the only remedy is lifting.

High insulin increases the growth and hardening of fat cells, which leads to increased inflammation in the fat tissue. Lymph drainage and blood circulation are impaired. In estrogen dominance, fat storage increases. Especially upper layer fat accumulation increases the orange peel appearance. Vascular permeability increases and this causes oedema. Sedentary life, prolonged sitting and decreased muscle mass impair circulation.
Mitochondria are energy factories. In impaired function, fat cannot be burned. Intestinal bacteria problems increase the endotoxin load. This increases inflammation in the skin.
Vitamin C, zinc deficiency, insufficient protein and excess sugar are other factors.

Considering this situation only as an aesthetic problem, especially devices that give heat and damage worsen the situation. First of all, it should be considered as a medical problem and aesthetic surgery procedures should be built on this.

What you need to know before treatment:
What Causes Cellulite?

We can explain the formation by a simple mechanism: Push and Pull.

  • Do not push: The fat cells under the skin grow and push the skin upwards.
  • Do not pull: The vertical bands of connective tissue surrounding the fat harden and pull the skin downwards.

These opposite forces cause fluctuations on the skin surface. Our treatment methods aim to correct exactly this mechanism.

Cellulite Treatment Methods

In our clinic, we apply combined protocols for the treatment of the source of the problem (fat, connective tissue or skin quality). Here are the methods we use most frequently:

Cellulite treatment methods infographic: SVF stem cells, exosomes, microstimulation and liposuction

Methods of cellulite treatment: SVF stem cell, exosome therapy, microstimulation and superficial liposuction infographic

1. Subcision (Subcutaneous Release)

It is the most effective method that we usually apply at advanced level. The hard fibrous bands that pull the skin down are cut free with a special technique and permanent improvement is achieved.

2. Enzymatic Lipolysis

We administer special enzymes that break down fat cells under the skin. We frequently use this method especially in areas with stubborn fat.

3. Mesotherapy (Cellulite Vaccine)

It is the injection of vitamin-mineral mixtures that accelerate circulation and support fat burning.

4. Autogenous Exosome Therapy

It is the process of improving skin quality with reparative cells obtained from the person's own blood.

5. Microstimulation Fascia Therapy

By stimulating the fascia tissue, we regulate circulation and increase tissue elasticity.

The Most Effective Surgical Cellulite Treatment Methods

The orange peel-like tissue concerns the superficial fat tissue. Liposuction can be applied to this area. However, if there is ‘false cellulite’ (skin sagging), standard treatment instead of Leg Lift preferred.

Grading in Treatment Planning

Staging is important for a correct treatment plan:

  • Grade 1: It only shows when it is compressed.
  • Grade 2: It appears when standing, disappears when lying down.
  • Grade 3: There are prominent pits both standing and lying down.

Lifestyle Recommendations to Support Treatment

No medical procedure alone is a “miracle”. To speed up the process, we recommend the following:

Expert Tip: Proper hydration is important in the treatment process.
  • Dry Brushing Accelerates lymph circulation.
  • Reduce Salt: Prevents oedema.
  • Strength Exercises: Tightens the skin by increasing muscle mass.

Is cellulite treatment permanent?

Medical treatments provide long-term results (1-3 years), but maintenance (reminder) sessions may be required with ageing and weight change.

Do thin people get cellulite?

It's not just about weight. It is not only related to weight; it is related to skin thickness and connective tissue structure. It can also be seen in very thin people.

Do cellulite creams work?

Creams moisturise the skin and provide temporary tightness, but they cannot remove the fibrous bands or fatty tissue under the skin.

Is there a cure for cellulite?

Yes. From a medical point of view, it is possible to reverse the stage and significantly improve the appearance. Subcision and Lipolysis are the most effective methods.

How does cellulite go away the fastest?

The fastest result is aimed with medical interventions. Especially with the Subcision method, the release of the recesses under the skin provides a rapid improvement.

What is the most effective cellulite treatment?

Combined protocols are most effective rather than a single method. Subcision for pitting and lipolytic treatments for adiposity should be used together.

Which hormone causes cellulite?

Primary role Estrogen hormone plays a role. Therefore, it increases during periods of hormonal change.

1 Does cellulite go away in a month?

It is difficult with diet alone, but a significant reduction in its appearance can be achieved within 1 month with clinical medical treatments.

Is walking good for cellulite?

Walking improves circulation but alone cannot break the hard bands. Medical support is essential.

How to remove cellulite on the leg?

It is planned according to the stage. In advanced stages, the bands should be opened with non-surgical medical interventions.

Does cellulite go away as you lose weight?

No, this is a common misconception. Even if you reduce fat volume by losing weight, you cannot eliminate subcutaneous bands.

What causes cellulite most often?

The main cause is genetic and hormonal. When sedentary life and malnutrition are added to this, the formation accelerates.

How to remove cellulite on the buttocks?

The most effective method for deep pits on the buttocks is the Subcision procedure, which allows the fibrous bands to be released.

What causes cellulite the most?

Refined sugar, excessive salt, packaged foods and prolonged inactivity are the factors that increase it the most.

Which vitamin deficiency causes cellulite?

Vitamin C deficiency, which is essential for the production of connective tissue, may impair the elasticity of the skin, making it more pronounced.

What is painful cellulite? Does cellulite cause pain?

Normally it does not cause pain. If there is pain with touch, this Lipoedema may be a disease.

The Culprits Behind Cellulite, Lipoedema and Waist Bagels in Menopause

Most patients also have lipoedema. Actually, the main diagnosis is lipoedema. In the pre-menopausal period, the female body generally tends to have a “Pear Type” structure (fat is stored in the hips and legs). However, with menopause, this structure rapidly transforms into “Apple Type” (fat is stored in the trunk and abdomen). In patients with lipoedema, apples and pears are seen together. Here are the main reasons for this:

1. Removal of the protective shield of oestrogen

Estrogen is not only a reproductive hormone; it is also like a “traffic cop” that determines where fat is stored in the body.

  • With oestrogen: The body directs fat under the skin (subcutaneous), to the buttocks and thighs.
  • Estrogen withdrawal: This guidance mechanism is disrupted. The body begins to store fat around the organs (visceral fat) and in the abdominal cavity. This is the number one cause of enlarged waist circumference.

2. Risk of Insulin Resistance

Estrogen decline can weaken the body's ability to process blood sugar. When cells are less responsive to insulin, the pancreas secretes more insulin. High insulin levels give the body a single command: “Store fat and never burn it.” This storage usually takes place around the navel.

3. Cortisol (Stress Hormone) Dominance

Menopause can be stressful due to insomnia, hot flushes and mood changes. The cortisol hormone secreted during stress pulls the fats in the body from the blood and fills the fat cells especially in the abdominal area. Because the fat tissue in the abdominal region is 4 times more sensitive to cortisol than other regions.

4. Muscle Loss (Sarcopenia)

Muscle mass naturally decreases with age. Muscles are the engine of metabolism; the less muscle you have, the fewer calories you burn at rest. Continuing to eat the same amount of food while the metabolic rate slows down in menopause results in inevitable weight gain (especially in the waist area).

Why Does Lipoedema Worsen in Menopause?

Lipedema (Lipoedema) is a painful and chronic adipose tissue disorder that is often confused with obesity but is completely different. It is characterised by disproportionate fat accumulation, usually in the legs and arms. Menopause is a critical threshold for lipedema patients and one of the most common periods when the condition worsens.

Why does menopause “exacerbate” lipoedema?

1. Hormonal Chaos and Inflammation

Lipoedema is a hormone-sensitive disease. It is triggered during periods of major hormonal change such as puberty, pregnancy and menopause. Fluctuations and eventual decline in estrogen and progesterone levels increase inflammation in lipoedematous adipose tissue. This causes patients to feel more pain, tenderness and heaviness in their legs.

2. Increased load on the lymphatic system

Visceral (belly) fat that comes with menopause increases intra-abdominal pressure. This pressure makes it difficult for the flow of lymph fluid and blood, which must return upwards from the legs.

  • Conclusion: The already sensitive lymphatic system is further strained, oedema (swelling) of the legs increases and the stage of lipoedema may progress.

3. Relaxation of connective tissue

Estrogen is critical for the production of collagen, which provides the firmness of the skin and connective tissue. When collagen decreases in menopause, connective tissue becomes loose. Lipoedematous tissue already has a “loose” structure; with menopause, the tissue loses its support, which leads to a more pronounced deformity and appearance of the legs.

4. Vicious circle: Movement Restriction

When joint pain and lipoedema pain increase during menopause, women may avoid moving. Decreased movement fuels weight gain and weight gain feeds lipoedema. This vicious cycle exacerbates the symptoms of lipoedema.

Conclusion and Recommendations

Although the change in body shape and exacerbation of lipoedema in menopause may seem like “fate”, it is possible to manage this process.

  • Menopausal bagels and lipoedematous fats are resistant to diet and sports. For this, these fats must be removed by liposuction.
  • Transdermal oestradiol therapy and progesterone  its administration is recommended in menopausal patients with lipoedema.
